While tooth decay and periodontal (gum) disease destroy more teeth than any other causes, both of these diseases are largely preventable with proper oral hygiene and dental treatment. It’s more than possible, then, to enjoy a lifetime of healthy, disease-free teeth.
But even with healthy teeth, the effects of aging will cause tooth wear over time. And although we can’t prevent the aging process from occurring altogether, there are steps we can take not to accelerate the process.
Most tissues, including bone and teeth, have a growth cycle in which older cells are broken down (known as catabolism), removed and replaced by newer cells (anabolism). As we develop during childhood, the growth phase exceeds breakdown; when we reach adulthood, the two phases come into equilibrium. But as we age, breakdown will gradually overtake growth. This aging effect results in, among other outcomes, tooth wear.
“Normal” wear appears to be greatest — and most visible — along the biting surfaces of the teeth. The forces generated when we bite or chew causes enamel to erode over time. Unfortunately, you can accelerate this process through bad oral habits: clenching or grinding teeth, often times at night while you sleep, as well as habitually chewing on hard objects like nails or pencils.
Normal forces generated when we bite or chew are actually beneficial for dental health — they help stimulate bone growth. But when they exceed their normal range as when we clench or grind our teeth, they can increase tooth wear and cause other problems such as diminished function or changes in appearance, such as a shortened facial height.
To slow the rate of wear, it’s important to modify any behaviors that may be contributing to it. In many cases an occlusal night guard worn while you sleep helps prevent teeth clenching. You may also need assistance with stress management, a major trigger for these kinds of habits, through biofeedback therapy or counseling.
If you’ve already encountered excessive wear, bonding techniques using colored composite resin, veneers or crowns that attach directly to the teeth can restore lost function and rejuvenate the appearance and color of your teeth. Periodontal (gum) disease is mainly caused by bacterial plaque built up on tooth surfaces due to ineffective oral hygiene. For most cases, treatment that includes plaque and calculus (tartar or calcified plaque) removal and renewed daily hygiene is highly effective in stopping the disease and restoring health to affected gum tissues.
However, you might have additional health factors that may make it more difficult to bring the disease under control. If your case is extreme, even the most in-depth treatment may only buy time before some or all of your teeth are eventually lost.
Genetics. Because of your genetic makeup, you could have a low resistance to gum disease and are more susceptible to it than other people. Additionally, if you have thin gum tissues, also an inherited trait, you could be more prone to receding gums as a result of gum disease.
Certain bacteria. Our mouths are home to millions of bacteria derived from hundreds of strains, of which only a few are responsible for gum disease. It’s possible your body’s immune system may find it difficult to control a particular disease-causing strain, regardless of your diligence in oral care.
Stress. Chronic stress, brought on by difficult life situations or experiences, can have a harmful effect on your body’s immune system and cause you to be more susceptible to gum disease. Studies have shown that as stress levels increase the breakdown of gum tissues (along with their detachment from teeth) may also increase.
Disease advancement. Gum disease can be an aggressive infection that can gain a foothold well before diagnosis. It’s possible, then, that by the time we begin intervention the disease has already caused a great deal of damage. While we may be able to repair much of it, it’s possible some teeth may not be salvageable.
While you can’t change genetic makeup or bacterial sensitivity, you can slow the disease progression and extend the life of your teeth with consistent daily hygiene, regular cleanings and checkups, and watching for bleeding, swollen gums and other signs of disease. Although these additional risk factors may make it difficult to save your teeth in the long-run, you may be able to gain enough time to prepare emotionally and financially for dental implants or a similar restoration.

When it comes to our overall health, many of us think we’re pretty well-informed. But a recent survey quiz given by the American Dental Association (ADA) revealed something surprising: When it comes to dental health, most people could use plenty of “brushing up.” In fact, the average score on the true/false test was a barely passing D! Among the questions most people answered incorrectly were:
- How often should you brush your teeth? (91 percent got it wrong)
- At what age should you take your child to the dentist for the first time? (75 percent wrong)
- How often should you replace your toothbrush? (65 percent wrong)
- Can cavity-causing germs be passed from person to person? (59 percent wrong), and
- Does sugar cause cavities?
We’ll come back to the last question in a moment — but first, let’s recap some basic dental health information.
While you might think it’s best to brush after every meal, the ADA recommends brushing just twice a day. That’s because excessive brushing can erode tooth enamel (especially if it has already been softened by acidic food or drinks), and can also expose and irritate the root of the tooth. But when you do brush, you should keep at it for at least two minutes each time!
Bring your child in to the dental office within six months after the first tooth appears — but no later than his or her first birthday! The age-one dental visit starts your child off right with proper preventive care and screenings, and sets the stage for a lifetime of good oral health.
Most people think it’s OK to change your toothbrush twice a year — but the ADA recommends that you get a new one every three months; that’s because stiff, frayed bristles just don’t clean your teeth and gums as well as they should. Likewise, most people don’t realize that the bacteria that cause cavities can be passed from one person’s mouth to another — by putting a child’s pacifier in your mouth or sharing a toothbrush, for example.
And speaking of cavities: Technically, they aren’t caused by sugar, as 81 percent of people thought. Tooth decay occurs when certain types of oral bacteria release an acidic byproduct that attacks the tooth enamel and creates small holes (cavities). This occurs after the bacteria have metabolized sugar in your diet. So while sugar doesn’t directly cause cavities, it does lead to tooth decay by feeding harmful bacteria. How about partial credit for that one?

There’s a new focus on children’s nutrition by both parents and schools; in fact, many school districts have instituted policies that encourage children to eat more nutritional foods and snacks. Regarding snacks in particular, the United States Department of Agriculture (USDA) recently released new regulations for the Smart Snacks in Schools Initiative that call for more whole grains, fruits and vegetables and less fat, sugar and salt in snack foods.
These regulations will help fight obesity and related medical problems like diabetes, but many dentists don’t believe they go far enough in one particular area — the consumption of sugar, a major cause of tooth decay. Dentists also feel the guidelines are too generous in the amount allowed for highly acidic beverages like sodas, ice tea, sports drinks and energy drinks that increase the risk of enamel erosion and tooth decay.
You may also be concerned about how much sugar your child is eating, and for the most part you’re able to manage their intake when they’re at home. But what can you do to influence their snack choices and habits when they’re at school?
For one thing, get involved with your child’s school and with other parents. Let school officials know your concerns about the sugar, fat and salt content of the snacks offered in the school’s vending machines and food service, and work to implement policies that discourage less nutritional snack foods. You should also set limits for your children about what snacks they can buy at school — along with explaining why they should avoid certain kinds of snack foods in favor of others. And, be sure to send healthy snacks along with them when they go to school that are bite-sized and fun.
It’s also important to help your children limit how often they snack and avoid “grazing” — nibbling on snack food for hours on end. Grazing can cause the mouth to be continuously acidic and never allow saliva an opportunity to neutralize the acid. You can also suggest similar policies to school officials, such as shutting down vending machines at certain times of the day.
Nutrition is essential to good health, in the mouth as well as the rest of the body. As a parent, it’s your job to see that your children eat nutritiously — enlisting their schoolâ??s help will make that job a little easier.
